To Apply for Membership:
1. Fill out the membership form (PDF). Please list all names for family membership.
2. Fill out the liability form (PDF). Waivers must be signed by all participating members and/or by parent/guardian.
3. Make membership fee check payable to NPT Membership and mail to NPT Membership
4. Mail two forms and check to to:
